Medical Information
Medical Forms
A complete medical history of your camper is the best assurance that we can offer the proper care for your child during the summer. Prior to the start of camp, we require each camper to have submitted his/her health history, insurance information, and physician’s form. Campers must have had a physician exam within the last 12 months in order to attend camp.
Perlman Camp’s policy is that all children and staff must be up-to-date on their immunizations.
Camper Medications
To ensure the best possible quality of care in our health center, we require all campers who take daily prescriptions, supplements, vitamins and/or over-the-counter medications in pill form to enroll in a pharmacy service. Perlman Camp has partnered with a medication vendor, Pack My Rx, to dispense and package all of your child’s medicine for camp. The licensed pharmacy will fill your child’s prescriptions and deliver them to camp in sealed packages that are marked and divided by the child’s name, dosage, date, and time to be given. All pill medications, including vitamins, gummies, herbal supplements, and over-the-counter antihistamines will be required to go through the medication vendor. The only exceptions are inhalers, epi-pens, nose sprays, injectable medications, prescription ointments, and refrigerated medication.
We do expect 100% participation from families with campers who will need medication in pill form while at camp. The only exception is if the pharmacy notifies us that they are unable to accept your insurance or fill a particular medication.
Please register online at their website. They will package your camper’s meds and deliver them directly to camp prior to your child’s arrival. All pills will be pre-packaged in dose/blister packs. The medication vendor will manage every step of the process, including insurance payments, dispensing and delivery.
